Start with a new image.  Mine was 800x200.  Fill the background with a med-dark color.

Create a new text layer
centered in the middle of the screen with the text you want to use.  Use a light color for the text.

Create a new Layer
and call it text_bg.  Move it below the text layer.

Hide the text layer by clicking on the eye next to it.

Hold down the CTRL key and click on the the text layer to select it.

Change your foreground color to a med tone.
Then, fill the selection with it by pressing ALT+BACKSPACE

Fill color: #195C9B

Deselect CTRL+D

If you have smaller text size (around 45-75) then just run the fragment filter on the text_bg layer.  Then skip to step 6.

FILTER > PIXELATE > FRAGMENT

If you have larger text (100+) then create 3 copies of the text_bg layer by dragging it to the new layer button
.

Set the opacity on all of them to 20%.

Move the top Layer copy 3 over sideways just a bit more then then width of the text itself so there is a small gap between the text.  
Next move copy 2 down the same amount, and then move copy 1 down and then over the same amount also.

Select the copy 3 layer, then press CTRL+E 3 times to merge the layers together.

Change the opacity of the layer to 100% then run the craquelure filter on it.

FILTER > TEXTURE > CRAQUELURE... ( spacing=15 depth=7 brightness=9 )

Set the Layer Effect Mode to Drop Shadow by right clicking on the layer, and selecting:  

EFFECTS > DROP SHADOW...
( distance=5 blur=5 )

Set the visibility on the Text layer to visible again.

Set the Layer Effects to on the text layer:
DROP SHADOW... ( distance=5 blur=5 ) BEVEL & EMBOSS... (depth=7 blur=5)

TIPS:: Try it with different fonts and different effects on the text on top.
